SUMMARY:UNC REX Mobile Mammography
DESCRIPTION:NC State employees can receive 3D breast cancer screenings aboard the UNC REX Mobile Mammography Unit when it visits Centennial Campus. To qualify for a screening\, you must fill out this form and meet the following requirements:\nYou must have a physician or a medical home. You can’t have a previous history of breast cancer.You can’t have any breast cancer-related symptoms\, such as pain\, lumps or nipple discharge.You can’t be pregnant. You must bring your insurance card and photo ID.The last day to register for an appointment is Sept. 8. When you fill out the registration form\, be sure to select NC Centennial Campus as your employer. More information about the mammography unit can be found on the UNC Rex Healthcare website.
